Consider surgery: If you have a partial rotator cuff tear as documented by an MRI scan, and you have had thorough conservative care (physical therapy, rest, nsaid's, and a cortisone injection), you may to consider surgery to address your ongoing pain.  Arthroscopic shoulder surgery should be able to take care of the problem for you.  Discuss with your orthopaedic surgeon re your best choice.

i have crps and now frozen shoulder. have had 2 cortisone injections in the space of 3 months. physio not helping. surgeon has no idea. what can i do?

CRPS: Complex regional pain syndrome (CRPS) is a chronic pain condition most often affecting one of the limbs (arms, legs, hands, or feet), usually after an injury or trauma to that limb.  CRPS is believed to be caused by damage to, or malfunction of, the peripheral and central nervous systems.

Neck: Need to see orthopedic md for evaluation. Cervical spine MRI may be indicated. MD can determine whether or not PT is indicated

Tell your doc: The sensation you describe of pins and needles in your hands and fingers is referred to as paresthesia and may represent irritation of the cervical (neck) nerves to your hand on that side likely as a result of the physical therapy for your shoulder. Tell you physical therapist and doctor before any more physical therapy. He or she may want to do more tests and or change the shoulder therapy.

Sometimes: In properly indicated cases, cortisone injections may fix shoulder pain.  Its usually helpful for pain from arthritis, and bursitis in the setting of an intact rotator cuff. Its not very helpful for rotator cuff tears, or labral tears.

Slows healing: Cortisone injections in the shoulder decrease the inflammation or arthritis or bursitis and tendinitis. This is good for pain relief and allowing for exercises. The side effects are occasional infections and increase tendon injury, because pain protects.

Calms inflammation.: Nobody uses cortisone anymore unless you are a horse.  Everyone knows what it is though so we usually call steroid injections "cortisone".  This is essentially equivalent to putting Ibuprofen right at the source of the inflammation.  The inflammatory response is a chain reaction in our body.  Steroid works a bit higher on the sequence of reactions and stops the inflammatory response.

See answer: Faciak flushing is a common side effect of steroids. This reaction is more common in women and is seen into up to 40% percent of patients. This can begin within a few hours of the injection and generally lasts only for a few days and on occasion longer depending how long the steroid remains in your system. No specific treatment is warranted.
