The virus usually causes the initial infection, and the patient is colonized with the bacteria that secondarily infects the middle ear. These diseases are transmitted by hand to mouth contact from another infected host, the same as a cold. Not from blood, food.

Strep treated?: Was the strep treated with antibiotics?  The lump on the neck needs to be looked at by a doctor, but is probably enlarged lymph node, especially if it is off to one side of the neck.  Any lump in the midline of the neck needs to be examined.  Follow up with your doctor, preferably the one who diagnosed strep.

The large majority of colds like your wife had, and your symptoms are typical, are due to viruses, which are not helped by antibiotics. Your wife probably was going to improve just as quickly with or without the antibiotic, and you probably dont need one. But check with your doctor if uncertain. Good luck.
