The : The best idea would be to call the clinics near you.  Start a list and write down what they say and ask specificic pricing questions (how much per office visit, how much per procedure, etc).  Once you find a clinic you are comfortable with, go in and meet with the office manager to clarify pricing for procedures and ask about any unexpected costs that may arise.

Not likely.: See recent impactful publcation in the new england journal of medicine [n engl j med. 2012 may 10;366(19):1803-13. Epub 2012 may 5]. The increased risk of birth defects associated with ivf was no longer significant after adjustment for parental factors. The risk of birth defects associated with icsi remained increased after multivariate adjustment.

Absolutely: Blocked tubes is the reason ivf was invented, and ivf is often a better approach than a surgical tubal ligation reversal. See a specialist (reproductive endocrinogy / infertility or rei md) who offers both options to assess your individual situation. Workup includes checking your partner's sperm (semen analysis) even if he's the father of your kids, and checking your egg supply. Best wishes.

???: Hysterectomy means removal of the uterus. If you have had a hysterectomy then you do not have a uterus to carry a baby. Check with your doctors to see what exactly you had removed.

Nope: You can try to spontaneously concieve using ovulation kits. If you are unable to get pregnant within 12 months, i would recommend other measures such as IUI or IVF

In your: Situation, the IUI has not worked.  Is the tube open?  At 37 the window may be beginning to close.  Discuss with your doc the pros and cons of another iui.  Good luck.

Try Again: Success rates for IVF are low in your age group. You may have to try again. Please remember, you also have an increased risk of miscarriages and birth defects. Have you considered using donor egg?
