Muscle spasms: Usually a muscle spasm happens around the spine when it tries to "stabilize" the spine during a strain. When you strain it during sleep in a bad position or when you turn abruptly the wrong way, the spasm lasts long after the uncomfortable event is done. In order for it to go away, spine needs to be realigned, and the muscles need to relax. Sometimes heat helps that or muscle relaxants or yoga etc.

Doctor can evaluate: A doctor at the E.R. Can evaluate to see if there is a neck injury, which can be quite dangerous (a risk of spinal cord injury). In general, possible spine injury victims should be transported by ambulance on a backboard, wearing a special neck collar. Afterwards, if only a minor sprain or strain, the physiatrist or orthopedic doctor can examine and refer one to physical therapy for help.

Simple trick:: Place 2-3 tennis balls in a large cotton athletic sock. Move neck slowly and slightly from left to right and then up and down. Advil and Aleve (naproxen) also work if not allergic to them.

Torticollis neck: Treat with heat packs, anti inflamatory and physiotherapist.  If does not help get stronger treatment  at your doctor.

Occipital neuralgia: See a neurologist, they can treat you with nerve blocks and/or medicines. Physical therapy may be helpful as well. Stay away from opioids. Good luck!

May need specialist: Painful/stiff neck in your age group may be related to spinal (C-spine) arthritis. So you may need to see a specialist such as a Neurosurgeon who will likely check you c-spine with X-rays to check for arthritis which can impinge on the spinal nerves leading to pain in the neck and muscle spasms. Try taking Tylenol(Paracetamol) and see if it might control your pain.
