my bloodwork says my wbc and rbc are high, mchc & mch are low, gran # (auto), ast and alt high. should i be worried?

Tests: Lots of test numbers. HIgh WBC count ususally sign of some type of infection, inflammation or stress. You have MCHC and MCH which are usually low with anemia, but high RBC _(dehydrated?). Your liver enzymes AST/ALT being elevated can be from a number reasons-dehydration, infection, inflammation , medications. All in all I would talk to your doctor

Possibly Autoimmune: Your symptoms are concerning for an autoimmune process. This is a process where the body attacks itself. The symptoms you give can also represent a serious, slowly progressive infectious disease. A general blood count is no where near enough to define what you have. You need to see your physician as soon as possible.

cbc auto diff shows  gran% = 80%, gran abs = 10.5 x10e3/ul, lymph% of 17.1%, & wbc of 13.1 thousand; all other results were normal.  dr. recheck when?

Depends on diagnosis: This mild increase of white cell count with 80% granulocytes in and of itself is not worrisome. It needs follow up with doc depending on clinical diagnosis. For instance if you have bronchitis that was treated and resolved, you may not need to repeat blood work. Since the clinical picture matters a whole lot here, it is best to follow advice of the Doctor who diagnosed your condition.

: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Talk to the doctor who ordered the tests. Having said that, how immature are the granulocytes? Even bands are immature granulocytes but are not abnormal in the numbers you mention.    Wish you good health!

: Yes, steroids can make that change.

Accepted Answer

High : Granulocyte counts points to stress or bacterial infection.  CBC interpertations need to be viewed in context of history and physical exam (go back to your doctor that ordered the test), and cannot deliver an answer in isolation.  Uncommonly some tumors cause such results. And sometimes it is normal on next draw.

Not likely HCV: It would be very unusual for active hcv infection to cause immature wbc.  Anemia definitely is seen in active hepatitis or with advanced liver disease.  If not a reaction to an acute infection or inflammation,  immature granulocytes usually point to a bone marrow issue.

Need diagnosis: You will need more tests to arrive at the diagnosis.

Not yet: If the absolute counts of both granulocytes and lymphocytes are fairly close to reference range, and all cells are well-formed, you are fine. Percentages fluctuate wildly over hours.

May be infection: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Having said that, high counts of neutrophils are usually associated with bacterial infection. It would have been helpful to know the actual count and any symptoms you may have.
