Yes: What you're explaining is acanthosis nigricans could be a sign of high blood sugar and diabetes (insulin resistance).  Some medications like growth hormones or birth control pills can also cause acanthosis nigricans. With weight loss and control of diabetes and lowering blood sugar to normal levels (less than 100 fasting and less than 140 random), a lot of times the condition resolves over time.

i have 2 small black spots at the base under my middle toenail.  i don't recall banging it but was stepped on once.  i don't know if it has grown?

Trauma: It is nit unusual for that discoloration to occur under a toenail that has been traumatized and it may last weeks. If you have more concern, consult a podiatrist for evaluation and advice.

i have a brown mark kind of circular shaped under my big toenail for the past 4 months which hasn't gotten any bigger or smaller which has 3 black spots inside the brown mark and i was wondering what it could be. i've had no injury to my foot at all?

Need examination : Lesions that present themselves under toenails need to be examined. I would go to your family doctor or if you have a podiatrist that you see.  Biopsy might be required to help diagnose the lesion.

Start of infection: She may have skinned her foot when she fell.Because she is a diabetic she is more prone to get an infection, therefore it is advisable to have her seen by her doctor to rule out the possibility of an infection.
