after wearing a 24htmj decompression splint for the last 6months& my teeth are in contact only on front. it is hard to eat, tmj pain.is it normal?

Anterior displacemen: I assume that your disc is still anteriorally displaced and you have closed locking of the jaw joints.  Hard to say without a history and exam.  I would certainly inform the dentist/specialist who delivered you the splint and ask for options.  I do some specialized treatment to conservatively get the mandible backward to try and get the teeth touching in the posterior again.  Hope they do too!

Bruxism: Is your splint store bought or professionally fabricated? If store bought, consider seeing a dentist who will fabricate an appliance far superior to what you now have. If your appliance has been made by your dentist, then you should be reevaluated to determine if the bite is correct or if other treatments are indicated.

Accepted Answer

Yes: If you are wearing a hard acrylic TMJ splint it is possible for you to break a tooth.  A tooth that is hitting prematurely on the splint and/or has a large filling in the tooth may be more susceptible.

why would a dentist give you a splint for tmj?

Break the pattern: There may be many reasons to use a splint and there are many types of splints.

One reason is to break a pattern of clenching or grinding.  The separation of the teeth stops the unconscience feed back loop/reflex habit.  Sometimes the teeth and bite are locking the jaw into a position that is restricted causing muscle tension.  A splint with its flat surface removes the restriction.

Probably, yes: It depends on what operation you have or had. With arthroscopy, you definitely need to wear your splint. With an arthroplasty, depending on whether or not the disc is removed or tied back, your splint may need to be adjusted. Your TMJ surgeon should advise you on whether or not to wear your splint or have it adjusted after TMJ surgery. If it's improperly adjusted, it may cause more harm than good.

Insurance for TMJ: Tmj surgery may or may not be paid for by either medical or dental insurance. It's more likely to be covered by medical than dental insurance. Dental insurance usually has a relatively low minimum and won't cover hospitalization. Your TMJ surgeon may need to provide your medical insurance carrier with appropriate diagnosis and procedure codes in order for it to be covered. We have had good results.

Tmj treatment: TREATMENT FOR JAW JOINT DYSFUNCTION AND PAIN SHOULD BE MULTI-FACETED and continue over many months. This could include home exercise, physical therapy, muscle laser therapy. The oral appliance is just one part. the type of oral appliance is important as well.

TMJ splint: There are three main types of bruxism splints: full arch hard splints, full arch soft splints, and the nti which goes over the front upper or lower teeth.
There are advantages and disadvantages of all three. Which splint is the best to use will be determined by your dentist based on your individual symptoms, including pain symptoms, and whether you grind or clench.

TMD\TMJ: Start by letting the dentist who fabricated the splint know that. It might require some adjustments or other treatment. Ask your dentist how well trained and experienced he\she is in this area and if you should be referred to another dentist with more training and experience. Most dentists simply make a night guard and cannot adequately treat more complicated cases.

Eval splint : How long have you worn the splint?   It can take some time for healing.  You may need to have  your splint adjusted.  A splint is so times used to, help diagnose a TMJ problem  so talk to your  general dentist or prosthodontist , who is  well trained in TMJ therapy, about the next step.
