Tooth decay: If the decay has entered the second layer of tooth structure, you have to get it filled. If the decay is confined to the enamel, only your dentist can tell, keeping it immaculately clean and seeing your dentist at least 2x a year may be sufficient.

Dr. : Dr. Zweig is correct.  Decay and cavity are just dummied down terms to the disease called  dental caries.  It happens when the acids produced by the bacteria which lives in our mouths eat away at our tooth structure and functionally dissolve it.
