Does OBGYN know?: Bleeding noted on underwear can come from several sources. One of the biggest concerns is bleeding from the uterus or placenta. An OB doctor would want to know about recurring or heavy bleeding.

It is also not unusual for them not to tell anyone about it.  Approach the topic sensitively and she if she can put you into the picture of what is going on. : Can you be sure this staining came from your vagina?  This type of vaginal discharge is usually from uterine shedding of old blood products and other tissue debris often left over from an old period. It doesn't sound serious but your gynecologist can advise you further if this staining or discharge continues.

This scenario you are describing could represent some irritation and/or damage with deep sexual intercourse with penetration.

If you period is late then it would be time to get a hpt.
