Does OBGYN know?: Bleeding noted on underwear can come from several sources. One of the biggest concerns is bleeding from the uterus or placenta. An OB doctor would want to know about recurring or heavy bleeding.

Vaginitis/cervicitis: Talk to your ob/gyn; this could be due to vaginitis, cervicitis, genital trauma, or even placental abruption (chronic, not acute). A thorough physical examination, ultrasound and microscopy analysis should yield the correct diagnosis and lead to the appropriate therapy. Refrain from sexual activity until this is resolved.
