Your primary care physician or a peer can guide you to care. The alternative is a miserable, long, gruesome death during which you are likely to tear off your own skin. BIG problem: Your question about being "incapacitated" without meth suggests you feel it supports you in some way.  This ignores the extreme risk to your health that daily meth use brings.  You'll need a team of addiction medicine and psychiatry doctors, your family doctor, possibly a dentist, and specific meth recovery program.  Depression can be treated; recovery from meth is challenging but very possible.
