Acne: Without seeing it, suggest you just keep clean and use a topical anti- acne cream. If it gets infected see your doctor.

See a dermatologist: You can't treat yourself with any effective otc medicine. It maybe a pimple, a infected cyst or abscess. Let dermatologist find the best way to treat it, he/she may find out what it is first.

New cystic acne: This sounds more like a row of abscesses.  My advice is to see your doctor as you may need an antibiotic.  Lesions on the face require visualization to be correctly diagnosed.

Irritable bowel: IBS or irritable bowel syndrome can be a miserable illness.  Hopefully you have a GI doc who can help guide you along.  You may be malnourished from all the diarrhea/pain/malabsorption you've suffered through. That could certainly affect hair growth and affect the immune system (weakening it) allowing acne to form.  Raw almonds are good for healthy hair. Keep hands away from face to reduce acne.

Let it be: Leaving it alone is the best option. You may try warm fomentation. Do not try to pop it as that will only lead to a scar. For good health - Have a diet rich in fresh vegetables, fruits, whole grains, low fat milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco or alcohol in any form. 
Practice safe sex. 
Get HPV vaccine.

MOST LIKELY COLD SOR: The location and description is more suggestive of a cold sore.Having said that it is very difficult to say with certainty what it is without looking at it.If you are concerned about it get it checked by your doctor

i am a female, 40's, taking 250 mg of cephalexin for over 5 years 1x daily for cystic acne with probiotic. should i be concerned with long term use?

I would: Antibiotics tend to lose effectiveness with time due to bacteria developing resistance to it. You don't mention how effective it's been for you, so more information in that direction may be helpful as well. Probiotic supplement is a must with the use of antibiotic, whether short or long term, but you may ask your derm doc if there are other options available in this day and age.

Forgive my frankness: Cystic acne is a brutal disease that gravely disfigures you now and will leave your skin permanently mutilated. You can't treat it on you own, and if your parents are interfering with your desire for effective therapy, pick up the phone and call your physician. Despite all the internet scare stuff, Isotretinoin is by far the most effective medication. It's not for sissies, but it's what you need.
