ic for 30 years. cysto & ct w/o con 2.5 years ago after grosshematuria-negative. cysto w biopsies showed inflamed, kub & ultra(just cyst) 3 mon ago after gross hematuria. 4 ecoli utis since. urine neg now except microblood esp later in day. is it ic?

Doubt : IC doesn't classically present with gross hematuria,   IC is a diagnosis of exclusion that presents with symptoms of dysuria,urgency&amp; frequency with a negative workup for other causes. Cystoscopically, the urologist may detect small pinpoint red dots of hemmorhage but microscopically it can look normal, or show inflammation and ulceration. Mast cells have been implicated with scant evidence.

E. coli is a GI bug: This almost doesn't quite make sense - for someone to have E. coli in the sputum means that something from the GI tract contaminated that. This can occur with aspiration pneumonia (a patient vomits and then gets pneumonia from that) or, rarely, there is E. coli in the blood stream that settles into the lungs. To me, it's a matter of matching the lab findings with what's going on with the patient.
