Complicated: When i was doing clinical medicine, these were collectively sort of the flags for sarcoid disease but I am not saying that's the case here. This is because sarcoid is a diagnosis of exclusion so the advice is to rule out other diseases with additional diagnostic tests which can be done by a primary care physician or rheumatologist.

No : No the high sugar will not mess with creatine or creatinine levels. You need your diabetes controlled however. Work on that with your doctor. Creatinine is usually measured in the blood while creatine is very seldom tested. The higher the blood creatinine the worse the kidney function, in general.

Creatine: absolutely not - dont worry about it - you dont need extra creatine and you don need to measure it.  In March 2015, a study published by the British Journal of Cancer found that men using supplements containing creatine may have a higher risk of getting testicular cancer .

Probably normal: Creatine kinase (CK), also known as creatine phosphokinase (CPK) is an enzyme made by muscle tissues, including the heart muscle.  Doctors most often are concerned with high levels of CK, which may indicate muscle damage.  "Normal range" varies by laboratory, but a level of 49 is probably within the normal range of most laboratories.

Lab is not a disease: You do not give us enough data. What you are describing is a normal creatinine (not creatine, i assume), BUN and calcium. Levels are normal. Levels also depend on the specific method used by the laboratory. Can't rule gn in or out without mre data.

Unclear question: Did the lab tell you that urine was too dilute for a valid test? If so you could limit your fluid intake the day before and provide first voided sample. What is your serum creatinine level?

: Nothing worrisome. Elevation is used as a marker for heart or skeletal muscle injury. Low is either not significant or  may reflect lower muscle mass than average. I would not worry about it.
