Good question: Low cpks don't mean much unless you're old and skinny. High cpks would indicate muscle inflammation or injury. You need to know the cause. Unless you have a muscle disease that is treatable, it will either normalize on it own or stay up.

Need diagnosis: High CPK indicates some degree of muscle cell damage or death, and is often an indicator of a muscle disease, (myositis or a muscular dystrophy or another type of necrotizing myopathy).  It may also indicate rhabdomyolysis which has a variety of causes.  Rest alone is not appropriate for any of these except, perhaps, rhabdomyolysis.  So you need to consult your doctors as to the cause of your cpk.

Inflammation: Usually a mildly elevated CPK and sed rate are hallmarks of inflammation somewhere in the body.

Chronic inflammation can also make you prone to being anemic, so get that checked out with some common blood tests.

Whoa: Myositis does not result from injury. If your CPK is up from a physical injury to your calf muscles, it's to be expected. If you exercise hard and often, your CPK will be higher -- often much higher -- than the lab's reference range in any case. If you do indeed have an illness in the "myositis" family of muscle diseases unrelated to fitness or injury, you will realize it.

: It is possible for the measured creatine kinase to come from muscles or the heart; the MB fraction is used to differentiate that coming from the heart and MM for that coming from the muscles. You are right that the MB fraction as well as Troponin helps to determine if there is heart muscle injury and alcohol may contribute because of tendency to injury to muscles and others like nerves and liver.

Muscle or heart : CPK  can be released from damaged or inflamed muscles, including the heart muscle.  Some muscle diseases that cause weakness and pain in the muscles can elevate cpk, as can any muscle injury (including an injection into the muscle).  Heart attacks and other heart muscle injury or inflammation can also raise the cpk.  There are tests to determine the source of the cpk. ("fractionation" of cpk).

my cpk tested 1885, 24hrs after lifting weights. 7 days later, w/o exercise, tested 3800. on 5mg/day of crestor (rosuvastatin) for 2+ years. never high before. why?

Statin: Unclear whether this represents statin myopathy or weight lifting effects on the muscle. Safest course likely to stop the Crestor (rosuvastatin) and watch what happens to the cpk. If it clears, a rechallenge either with Crestor (rosuvastatin) or with less potent statin like Lovastatin with close CPK observation may clarify what's likely.

i'm 25 yrs old. after lifting heavy my creatine kinase levels were 1800. when i don't exercise theyre normal. is this ck normal for young adults?

Good for you: From time to time, i've run ck's on classes of students. The bodybuilders, after lifting, can run as high as 3000.
