Pernicious anemia ,: How was your pernicious Anemia diagnosed ? Are you on B12 injections ? are you receiving Folic acid tabs ? If you are not receiving replacement therapy for both , you need to see a blood specialist for eval &amp; treatment .
A far as you Hgb dropping twice , there is  a source of blood loss , from your GI tract ( stomach or colon ) .you need EGD &amp; colonoscopy to look for blood loss.

Yes except heart: B12 deficiency can cause a variety of neurological symptoms including &quot;derealization&quot;; it can adversely affect brain function but that damage should be reversible. It can cause high homocysteine which increases risk of cardiovascular disease ; anemia puts stress on hearts, but it should not directly cause heart problems in someone your age. See

Not pernicious: anemia. The test you described is for a coagulation disorder, not for pernicious anemia. There is contradiction in the statement you provided and you should discuss it with the doctor who ordered the test.
Wish you good health! - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex.

Some: Pernicious anemia is due to malabsorption of vitamin b-12 from the GI tract.  It is a form of low blood cell count with associated symptoms of fatigue, paleness, breathing difficulties, etc.  While not very common, all such patients should be evaluated by their doctor to give treatment for the pernicious anemia if that is the only problem and to discover any other problem.
