Perhaps: If the herpetic lesions are open you may see blood on the surface of the stool or on the toilet paper. Blood in the body of the stool usually comes from a source higher in the colon which allows time for the blood to be incorporated in the feces.

To cut or not to cut: A fleshy tag around the anal opening present for 7 years without changing is nothing serious but can present a problem with "clean up" after a bowel movement.  Such tags can be removed surgically but it's a pain in the butt!  sorry for the pun.  Your choice if, it bothers you enough get it off otherwise no problem.

Rectal lesion: If it has been there for 6 years and not caused any problems, then it is mostly benign. The flap of skin could be a skin tag, polyp, or hemorrhoid tag. An examination would give you a definitive diagnosis.

Common symptom: Your symptoms are common, and I suspect you may have hemorrhoids. Even if you are not having pain, you can still have hemorrhoids which can be irritated when you have a bowel movement and can cause bright red bleeding in your stool. You should share these symptoms with your regular PCP. An in office exam can provide a quick easy answer, and provide you with treatment options if needed.

Anal fissure/piles: Your symptoms are consistent with anal fissure and/or piles. You should see a doctor as blood loss can lead to anemia. To keep you stool soft,  take a fiber laxative like Metamucil, drink enough water daily so that your urine is colorless. Blood in stool: Most common cause is hemorrhoid (internal or external). Next most common is a viral or bacterial infection. Next is a tumor in the colon or rectum &amp; a colonoscopy is needed. Needs attention: can you get a second opinion from a specialist? This area unless corrected fully, life long problem will arise.

It is not probably anal fissure.Keep the area as clean as possible.Make sure stools are soft.

may use hemorrhoidal cream if needed.

Soften stool/diet,,,: Likely the best way to stop this is by getting more fruits and vegetables in your diet.  This way your stools may be softened and so there'd be less pain with bowel movements.  The stress would also likely be reduced.  If there is still pain/stress with movements, you might want to see your doctor for an exam and recommendation for otc stool softeners or get a prescription for one.  Good luck.

Surgery: It may be time to get the lesions treated surgically.
Take oral iron and multivitamins to prevent anemia from blood loss.
