Scalp tenderness: If your scalp is causing you pain, it is unlikely that cortisone will relieve it.  There are may thing that could be causing it one of which is herpes zoster or shingles.  I certainly think if this is new onset you should be evaluated asap and even if this is chronic it warrants evaluation.  So please seek out your primary care physician or dermatologist.

Yes: Best used for persistent cradle cap , eczema, or bug bites.  Do not use for ringworm.

Deep breath: Is the itchy scalp causing the depression  or tis the depression causing an exacerbation of the scalp. 
Go to a new derm. Get a bx of the scalp. You may have seborrheic dermatitis that has a different treatment regiment.  If the scalp is cured and you are still depressed you need to treat the depression. There are many options.

: Without exam hard to tell Pl see a dermatologist at an academic center you might need a biopsy for accurate diagnosis and treatment

: No OTC steroid containing shampoo by my knowledge. You should use Selsun shampoo for dandruff, there's no need to use hydrocortisone ointment for the scratches unless there is significant itching or oozing.

my 7 year old has a circular patch of hair missing. scalp was red last week but hair in tact arnp said put hydrocortisone on. now hair gone not red ?

Hair loss: did the hair fall out after putting hydrocortisone on? Was the entire scalp red or just a small area? Patchy hair loss may be due to a fungal infection among other possibilities. The fungal infection is called tinea capitus and would need prescription medication. See your pediatrician for an exam and treatment plan.

my dr prescribed me locoid (hydrocortisone butyrate) & xolgel for my face & scalp. do i apply them at the same time? she only stated use once or twice a week

Medication: Both drugs can be mixed together.However, you should check with your doctor as to when to apply the creams. He may have wanted you to apply the Locoid (hydrocortisone butyrate) once or twice a week and the xogel to be applied every day.
