Tenesmus: That feeling is called tenesmus. Could be rarely from inflamed bowels, but most the time it's just from having diarrhea. If persisting  seek medical help.

Uti and bm: Good question!  Since your bladder and rectum are close to each other, a uti will cause irritation to your bladder give you the sensation of needing to urinate again after you've already done so. It is possible your rectum can be irritated, too. Have your PCP check you out. If he starts you on antibiotics, check in the rectal irritation resolves when the uti resolves. If not, may need GI consult.

i constantly find stool in rectum even after bowel movements. even if my rectum is full i wont feel the urge to go. is it internal rectal prolapse?

"rectal prolapse " ?: Unless one has had multiple and traumatic vaginal deliveries (unusual at age 21),  the natural tendency to withhold bowel movements due to psychological inhibition or unavailability of restroom facilities may cause stretching of the rectum which decreases the signal indicating a need for evacuation.  Ask your physician about medications to assist with constipation, and drink more liquids.

Exam needed: You give a potentially valuable clue when you mention the rectal involvement and lower back discomfort in your symptoms. In my opinion,this would tend to pinpoint the problem to be either in the rectum or prostate gland. I recommend an examination by a physician,proctologist or urologist to start the investigation.

LLQ pain: The more likely diagnosis here is diverticulitis,  especially with a colonoscopy that didn't show precancerous changes but had diverticula 17 months ago.  A good exam with a physician could confirm this suspicion and get you on the mend.  Please see someone.

Check with your doc: This doesn't sound typical for ibs - usually that's a pain that improves after a bowel movement as well as periodic constipation or diarrhea.  It would be worth seeing your doctor as this could be an inflammatory bowel disease like crohn's or ulcerative colitis.
