left knee pain started 7 days ago after running. dull ache, occasional sharp pain, hurts to put pressure around knee cap or on sides. fairly constant ache last 4 days. recommendations?

Heat/ ice pack: I presume you have tried ice pack then heating pad.  Have you tried NSAID?  It is also possible that you may have ended up with knee injury  or just the beginning of osteoarthritis.  If the problem persists or gets worse, it is best to consult an orthopedist.

Probably overuse: Since this involves both knees, it is probably from overuse, especially since only the back of the knee.  Take more frequent breaks to get up and walk around.  Perform simple range of motion activities by moving the knee through full range of motion and stretch at the limits of motion when fully straight and fully bent.  Ice may help if pain persists.  Apply ice to painful area 15-20 min/hour.

Get examined: Difficult to say without getting a more thorough history of your knee pain and performing a physical exam. See your ORS and you will know whether it's tendinitis, a meniscal tear or something else. Best of Luck!

Knee pain: The knee can be caused by arthritis, ligament injury, tendon tears, muscle strain ...  The treatment of knee pain depends on the cause of knee pain.

See MD: Depending on if there are associated symptoms, locking, clicking, swelling, etc. you may need to see a doctor.  In general, knee pain is relieved by quad and core strengthening.  Anti-inflammatory medications and ice as well.  A knee brace to control movement of your kneecap can help.  I would recommend seeing a Sports Medicine doctor or Orthopedist to determine the exact problem and get treated.

See Ortho if Needed: Usually most pain resolves on its own, however if it persists or you have swelling noted you may want to see an orthopedic doctor to evaluate it further.  Use Ibuprofen or aleve (naproxen) around the clock for the next several days to help as well.

Go see an orthopod!: People with acl tears can be asymptomatic for years.  However, it is not uncommon for people with tears to put lots of stress on the medial meniscus which can result in both acute tears and chronic wear.  Get evaluated before you do too much damage to the joint surfaces!

Try an OTC support: Most drug stores now carry elastic braces for all joints and the back even. You can try wearing one awhile and perhaps the support  will allow enough rest to heal whatever is strained.  A GP visit if you feel well understood might lead to good ideas and you might find some help in acupuncture  or therapeutic massage or physical Tx to rebuild support muscles.

mild but constant knee pain for 5 mnth on accutane, dose increased for 2 mnth,  pain has gotten worse. is permanent damage possible? should i stop?

Better stop: The accutane and see your doctor, hopefully no permanent damage, but you want to not add fuel to the fire, side effects of accutane can be serious, hence the need to keep close follow up and monitoring
