Reflux: One of the common reasons for your symptoms is acid reflux.  Reflux can cause irritation in the throat (even if you don't taste the acid).  Try treating the reflux and see if that helps.

This : This is likely due to eustachian tube problems caused by the sinus infection.  The antibiotics will clear the infection and help with the inflammation.  The nasal spray is possible a steroid spray to decrease inflammation, or a decongestant to decrease edema.  The positional component to the symptoms could be due to mucous in the middle ear.  Sometimes ear symptoms take a while to clear even after the sinus issues are over.

NO: Get an ENT exam and switch to another nasal cortisone ( steroid spray) Flonase or Nasonex or budesonide. Ask about a daily allergic inhibitor ( Singular) and a daily oral antihistamine ( Zyrtec  Xyzal or Claritin. You will need allergy testing and CT of sinuses as well as nasal endoscopy

It is a.Combination antihistamine and phenylephrine (decongestant).
