Fibrin in cyst: Ovarian cysts frequently will have stuff inside. When they do not, it is a simple cyst. When there is bleeding in to the cyst, they become complex. This causes all kinds of findings on ultrasound . Blood clots look like clumps and fibrin can cause stranding . All usually clear up

Follow-up required: In a patient of your age, it is likely self-limited and very well could represent a hemorrhagic cyst. However, a repeat ultrasound 6 weeks following the first is suggested to document resolution or decrease in size and to re-evaluate for patency of the ovarian artery.

Painful?: It looks like you have a GYN who is doing all of the right tests.  So what does she/he recommend?  In my practice, I recommend surgery for cysts this size that are painful.  Cysts that are not can get serial ultrasounds.  Most of my patients with cysts this size have opted for surgery since pain from these masses can escalate if they enlarge. I bet not, but...: I'll bet it's a mucinous cystadenoma. You do need to get it worked up.

Deep breath: Take a deep breath, no need to panic yet. It is 100% appropriate to follow a 62 Y.O. W/ a complex cyst, some turn out to be cancers but many do not. Follow-up ultrasound is critical, but may turn out to be nothing. Blood testing tumor markers is sometimes helpful but not always, and should only be done with a doctor who understands markers and what they mean. Please don't panic and best wishes!

Depends: How big it is, and if it is causing sxs and assuming they think it is benign and not malignant and obviously will need close followup.

Possibly: Ovarian cysts come in many varieties. The majority are benign. Complex features are more concerning and warrant further evaluation. This could be repeat imaging, blood work, or surgery. The evaluation would be determined on other risk factors as well. A gynecologist should be involved in the evaluation.

: If a "complex ovarian cyst" has been determined to exist, it is needful of a biopsy, usually done via laparoscopic surgery.  Contact your GYN physician to schedule this, as complex ovarian tumors require immediate attention.

See OB/Gyn : There are a number of causes for having a complex ovarian cyst seen on imaging, one of which could be potentially due to cancer. It is wise to have the appropriate imaging to monitor the growth and possibly a biopsy performed by your OB/Gyn. Make an appointment to discuss this further. Best wishes.

Complex ovarian cyst: The size of "2 cm" is less likely to be a malignancy, but any complex ovarian cyst should be sampled to make certain. This can be done by a simple laparoscopic surgery, and you should ask your gyn physician to recommend a referral to a gynecologic oncologist for expert advice and a second opinion.
