Alcohol is toxic and: potentially damages everything it touches, including hearts, but also livers, kidneys, bladders, etc.   Cocaine amps up everything (its a speed), plus is also toxic, so yes, it can damage tissue, too.   Heroin fills up opiate receptors and in effect, turns a lot of functions off.   Whenever you have tissue damage, you are likely to get inflammation, which is one cause of arterial plaque.

Both!: It depends on what your body responds to the most. Both drugs are highly addictive and both are extremely difficult to stop using. One body may respond more to one of the substances and another body to the other one. However, neither drug is something to fool around with because of the detrimental effects on the body, brain and life of the addict.

They've always been: There were actually a greater proportion of our population using opiates and Cocaine at the turn of the 20th century (the late 19th century and early 20th) than there are now.  Actually, these drugs are much less expensive than they used to be, with heroin being available for as little as $3 a bag (about 20 mg/heroin) and Cocaine at $25/gram. Abuse of prescription opiates has increased heroin use.

Variable: Abuse of any of the drugs can be deadly, but cocaine is not an opioid, but rather a sympathomimetic amine which can provoke heart attacks and stokes, killing in that fashion.  Heroin can cause apnea, but so can any other narcotic in sufficient dosage.  All are clearly addictive.

Too subjective to do: People's reports of 'withdrawal' vs. Objective measurements (vital signs) - what a mess. 
Heroin withdrawal is very painful/achy and people hate it- but it doesn't 'kill'. Yet a sudden withdrawal from alcohol could cause memory loss and seizures - far worse in my opinion.  Depending on what you call extreme, i would say alcohol !

Yes: You can die from taking a little heroin.  Any amount of heroin is too much heroin.

Effects: Simple answer. ..  Nothing good. All bad and no good.
From distorted veins, to loss of family, your own dignity, Jobs  and responsibilities, to stealing and robbing on to bigger crimes

which are too numerous to count.

Death: 'overdosing' on any substance (including alcohol) can kill a person. This especially includes heroin. Use of heroin is associated with a variety of severe emotional and general medical difficulties.

Accepted Answer

Too many to count: Firstly, there is no pure heroin - it is cut with all kinds of things, many of which can be dangerous.  Secondly, it is not sterile, and people who inject it may be injecting bacteria and fungus into their bloodstream, and if they share needles getting hep b/c or hiv. Overdose with death or brain damage, and becoming physically dependent (hooked) with painful withdrawal are the worst things.
