One : One with this history is susceptible to staph and other organisms being resistant even to Clindamycin so it is worthwhile to get cultures in this set up as well as general medical monitoring for progress.     Most importantly though is you would benefit from inpatient care for your chemical dependence.   Don't let it bother you if you've already tried that once or twice or more.   It's a matter of keeping at it, and then keeping at it.  
Hopefully, you can also be screened for other infectious agents that come from using IV medications and co-morbid medical and psychiatric conditions that can and do interfere with your recovery.  I'm glad you were willing to ask for help..Keep at it!

Get a proper Dx 1st: About 60%  of sore throats are viral & require no meds. If you take old meds every time you feel ill, you increase the creation of superbugs in our world with minimal benefit to you. Strep can be verified with a quick test & treatment with penicillin is 100% effective.

Not sure: Have you had previous reaction to latex, or local anaesthetic?  Could be a reaction to the manipulation of your lip during the procedure.   Don't think you need an antibiotic.  Return to your dentist for an evaluation.

Probably: clindamycin, or other antibiotics as ampicillins or tetracyclines, may decrease BCP hormonal efficacy, so better use an alternate contraceptive while using an antibiotic and a hormonal contraceptive method, that doesn't mean you stop or hold the BCP, please continue, best wishes

In a perfect world: You would take Clindamycin every eight hours to optimal spread the medication out and keep sustained therapeutic levels in the blood with minimal highs and lows, but that's easier said than done.

: This would be unusual treatment. It likely would work, but it usually isn't necessary to treat cellulitis for more than 10-14 days, and clindamycin carries higher risk of potentially serious side effects than more usual antibiotics, such as cephalexin (Keflex) or sulfa-trimethoprim (Bactrim). But depending on the speciific cause, clindamycin may be the best choice. Discuss with the doctor.

Depends: The dosage range is 10-30 mg/kg/d which places this at the high end of recommended dosing. If the infection is a deep or severe one it might be the best choise,With a lesser infection I might go with half that dose.
