If your surgical amputation is not healing well, you need to follow up with your Dr.

Depends: That depends on several factors like medical condition, blood sugar, circulation and your definition of healing.  Most below knee amputations are in a temporary prosthesis by  6-8 weeks but that can vary widely.

The cause was worse: Hopefully now that the offending limb is gone a body's response is first of focused healing, rather than fighting the greater insult/threat  of infection/gangrene/ischemia it was dealing with before the amputation.  
Phantom pain seems to be associated with the duration of the pain prior to amputation.   Physical therapy is critical to avoiding bad habits caused by limb loss.  Pursue rehab.

Usually no: Most amputations are performed under general anesthesia, though, spinal or epidural anesthesia is certainly fine if felt more appropriate by anesthesia.
