Infected/obstructed: Pyelonephritis refers to a kidney infection, whereas hydronephrosis refers to obstruction of the kidney leading to dilation and swelling.  You should see your doctor and possibly a urologist for both of these issues to get them evaluated to see if an anatomic abnormality or stone is contributing to infection or to see cause/severity of obstruction and if corrective surgery required.

Chronic Pyelo: Chronic pyelonephritis is the result of any process in the kidney that causes prolonged inflammation and fibrosis (scarring). Common causes include: persistent or recurrent renal infections, vesicoureteral reflux (urine going backwards into the kidney), or urinary tract obstruction. A board certified urologist or nephrologist are the best doctors to review these types of findings.

Stones or kidney?: If stones are cause of chronic pyelonephritis, then discuss removing stones to reduce risk for repeat infections as a means to improve kidney function. Optimize blood pressure & sugar as these are two main causes of renal failure. We don't usually remove kidneys even in failure.

Call the prescriber: If she forgot to tell him/her that she had any problem with ciprobay before, an alternate medication would be prescribed, otherwise the prescriber made a decision to treat with the most appropriate antibiitic for the case at hand, wish yourself and your wife wellness

Sediment: Infection means bacteria growing where it should not be. Your test for infection in your urine was negative and that is good. Sediment in the urine can be from infection, HOWEVER, there are other causes of sediment in urine that are benign. Try increasing your oral hydration, add lemon tea as well as water and juices to goal &gt;60 oz for a few days and see if the sediment improves. Be well.

No: Having your period or not will have no effect on duration of your kidney infection. Much more likely for kidney infection to affect the timing or duration of a period. Most kidney infections treated with 10-14 days of antibiotics. Duration of symptoms variable depending on infection severity but not uncommon to take a while to get your energy level back and feeling 100% again. Monitor for relapse.

Self Diagnosis: You "think" you have a kidney infection, but it could be something else. If an infection, you'll need an antibiotic. No OTC antibiotics. Your physician's office has taken precautions to help prevent the spread of COVID. You will not catch COVID during your visit. Failure to treat can cause more complicated issues.
