Yes: Can be chronic infection. It can be because of resistant organism, obstruction ....Stricture, stone , tumor, ......Immune status.

Kidney infection: The answer is yes. The first order of business is what caused the kidney infection in the first place. When it is not cured after "all the medication", it may be time to look for other causes.
