Infected/obstructed: Pyelonephritis refers to a kidney infection, whereas hydronephrosis refers to obstruction of the kidney leading to dilation and swelling.  You should see your doctor and possibly a urologist for both of these issues to get them evaluated to see if an anatomic abnormality or stone is contributing to infection or to see cause/severity of obstruction and if corrective surgery required.

Chronic Pyelo: Chronic pyelonephritis is the result of any process in the kidney that causes prolonged inflammation and fibrosis (scarring). Common causes include: persistent or recurrent renal infections, vesicoureteral reflux (urine going backwards into the kidney), or urinary tract obstruction. A board certified urologist or nephrologist are the best doctors to review these types of findings.

i have chronic pyelonephritis and four kidney stones in my kidney. it has shrunk immensely in size and is now in early renal failure. should i remove?

Stones or kidney?: If stones are cause of chronic pyelonephritis, then discuss removing stones to reduce risk for repeat infections as a means to improve kidney function. Optimize blood pressure & sugar as these are two main causes of renal failure. We don't usually remove kidneys even in failure.

Elm iron: Ic is a chronic inflammation in the "wall" (interstitium) of the bladder resulting in pain, frequency and blood in the urine.  Reportedly more common in women, it is probably well under diagnosed in men.  Best managed with dietary/lifestyle (hydration), there is only 1 medication approved by the fda - elmiron, (pentosan) to "fill the cracks" in the bladder lining. Intravesical (in the bladder) txs available.

can i develop pyelonephritis & chronic uti from postpolypectomy/peritonitis post-colonoscopy?

Complicated: If the intra-abdominal infection involved the kidney in some fashion then pyelonephritis could conceivably occur, but this should be treatable.  With this process the presence of a Foley catheter during your management can predispose to uti, as can bacteremias which occur during the infectious process.  None of these would necessarily create chronic uti, but anything is possible.

i was prescribed ondansetron 4 mg and cefuroxime 500 mg as i have pyelonephritis. i have developed a headache since taking my first doses of both drugs. can i safely take acetaminophen 1000 mg for relief?

: Yes, you can take tylenol for the headache and you should consider holding off ondansetron unless you throw up the antibiotic without it. Good luck.

what is the best solution for chronic reoccurring kidney infections?

: You need to find out if you have an anatomical reason to have recurring kidney infections. You should see a urologist to evaluate your kidneys, including imaging studies to see the structure of the kidneys as well as laboratory studies to see the function of your kidneys. If there is an anatomical abnormality resulting in these infections, this may need to be corrected. Good luck!

Kidney infection: Kidney and bladder infections are very different. They are treated in a similar manner but a kidney infection is associated with high fever and one sided back pain near the ribs. A bladder infection is associated with low back pain, frequency of urination/pain with urination and low grade fever. An untreated bladder infection can lead to a kidney infection or a kidney infection can be blood born.
