Prob not ur kidney: How are you so sure it is your kidney that is hurting.  More likely when you bear down to move your bowels, you are irritating a flank muscle or other muscle on your right side.

Lesion right kidney: This finding requires further characterization.  If this was an ultrasound finding, then the choice is to do no further testing, vs repeat ultrasound in 4-6 months, vs undergo MRI with Gadolinium.   This may be a simple cyst.  Harmless.  It may be a complex cyst, which is uncertain until further characterized by MRI.
