Murmur. : If the doctor heard a murmur, but the echocardiogram was normal, there could be two main possibilities. First, it could simply be a physiologic murmur which is of no pathologic consequence. Second, the echocardiogram was not sensitive enough to pick up the reason for the murmur. Regardless, you may want to follow up with the cardiologist to be safe.

CP: P wave flattening out during exercise, does not indicate ischemia. You may want to do a nuclear stress test or a cardiac cath after discussing with your cardiologist for a definitive answer. It especially indicated if you are a smoker have other risk factors.

Obstructive Sleep: Apnea is a condition of cessation of breathing during sleep that lasts for 10 seconds or longer. OSA should first be confirmed with the proper medical sleep studies. Regardless, the cause, could be only muscle pain or anxiety. However, pain in your chest with the symptoms you have described is a serious medical condition. It is always a red flag and appointment with your PCP or cardiologist.

This is worrisome: Any pain that occurs while sleeping is dangerous.  You need to have your heart examined as well as your chest, and soon.  Always sleep with your head up on a wedge or several pillows and see checklist for avoiding heartburn.  Good luck.  Get seen.
