Injury or pleurisy: Costal or muscular injury of the thorax can emitt a sharp or dull chest pain.  This may be caused from atheletics or unusal work.  Pleurisy (a rubbing or sharp chest pain on inspiration) can be associated with an infection (pneumonia), a ruptured lung bleb, or unusal rare causes internally.  Physical examination usually diagnoses the problem, and determines if a chest xray is needed.

You may have: pleurisy or an inflammation of the muscles and bones of your chest. Fibromyalgia? It is highly unlikely to be cardiac. If it persists after taking an anti-inflamatory like Aleve or Motrin you should see health care provider for further evaluation.

Anxiety: Your symptoms are compatible with depression/anxiety.   Talk to your doctor about your conditions.  Psychologic counseling can be very helpful. Meditation may also help reduce anxiety. 
In the meanwhile, no caffeine .

Many possibilities: How long, worsening? any trauma? any fever? these are the things we need to know before attempting a differential. It could be a lot of things from a broken rib, to a injured spleen, to diverticulitis. Bottom line: You need to see a doctor.

Seek treatment: Do you mean you have a wound infection? If you mean you have another infection elsewhere, it probably isn't related directly to the craniotomy. I would talk to your doctor if you aren't getting better. Chest pain and breathing problems could be signs of something much more serious, such as a pneumonia or a pulmonary embolus.

Please be seen: Some of that sounds like pleurisy. However, these are signs and symptoms that could suggest a serious infection including pneumonia. Some heart problems like pericarditis (inflammation of the heart lining) can also cause these kinds of symptoms but is less likely. Best to get a good history and exam to decide.

Why the concern?: Chest pain on deep inspiration only is almost certainly chest wall pain and is no concern. All young men have at least one enlarged neck lymph node. A temperature of 37c isn't a fever. Different people have different-shaped bones on their chests. Your dentist can advise you about your gum disease; i'd urge you simply to be grateful for good health and to continue enjoying your youth.
