what is it called when your vertebrae don't stay aligned when you tilt your head down? my x ray shows that the 5th cervical vertebrae slides out of alignment. is this something to be concerned about? dr recommends fusion?

: If you possess an unstable spine, especially in the neck, there is a risk of spinal cord damage, and this will require intervention.  Would utilize a cervical collar at this time, and also obtain a second opinion from a neurosurgeon prior to proceeding with fusion.

Fusion: A cervical vertebra can fuse for many reasons.
They can be that way from birth (congenital)
They can occasionally fuse from long term disc collapse (spondylosis)
Or they can be fused with a surgical procedure (cervical fusion surgery).

Many pillow promises: I have degenerative changes in my neck.  Different  things are going to work for different people. Learning to sleep on your back is the best thing to do. I was a stomach sleeper and had to learn back/side sleeping. Then you need o find a pillow that fits your body to keep your neck straight at night. I use a memory foam neck pillow, but your needs will depend on your body.

Normal: it is pad of fat you could feel it because it locate on the top of last cervical vertebrae  spine , it is normal just ignore it

An abcess is: a collection of pus. It doesn't matter where it is. The location just may make it more difficult to treat as it may be harder to drain ...

Not sure : I am not sure what you are expecting from the above. The diagnosis is listed - cervical degenerative disease. Vague area of marrow hypointensity is a very non-specific finding that i would not worry about provided there are no other findings on the MRI and they don't have a reproduction on the physical exam.

Hurting neck: What did you do to get that hurt?  Fracture?  Abscess?  Strain?  Better see your primary care doctor about this soon.  May need an x-ray or CT or MRI.

i was asked to go for a.p.& lateral  of the cervical spine.   the result was that  cervical spine is normal.spondylotic  changes are seen in vertebrae?

Spondylosis: Spondylosis or spondylitic changes are difficult names for arthritis or early degenerative changes in the spine. Very typical, and early on, rarely responsible for pain as they are normal sign of getting older (sorry!).
