Stones, stones: Kidney stones serve as a reservoir for bacteria since the local mileau of the stone is not real optimal for antimicrobials to work.  Get rid of the stones if you can and the infection may be able to be eradicated.

Fever: This is often characterized as a body temperature that during a 24 hr period either reaches 100F or fails to get as low as 98.6F.  The temp of 99.3F you quote is normal.

Facilitate: Stones do not cause infection, but make it more likely for infection to occur.
For good health - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex. 
Get HPV vaccine

IF there is: Obstruction this is a high risk situation for development of infection.  A serious infection could cause low blood pressure.  The other possibility is low blood pressure related to pain medication for the stone.

Ask Doc...: Inserting stent is an essential part of managing renal/ureteral stones for most of cases. But it comes with a price of local irritation and suffering, and occasional symptomatic uti, which can usually be managed and soothed with proper use of medications,  while the endoscopic procedures have averted patients to undergo open surgery. For individual details, ask attending doc timely.

: No it is not easy for a well trained Urologist to do that. During the procedure he/she will frequently check for the position of the stones and aim the sound waves at them.

: Delay in treatment of a documented "kidney infection" by "3 days" should not result in adverse effects on the kidney(s).  Many people mislabel a simple urinary tract infection for a "kidney infection".  A true "kidney infection" will have more defined symptoms and lab work will be usually abnormal.

Yes: It may be extremely difficult to successfully eradicate UTI with kidney stones present, both because they are a form of obstruction and infection occurs behind an obstructed focus as well as because bacteria will adhere to a foreign body like the stone and form a protective coating over themselves that protects against antibiotics and WBCs and antibodies.
