Bell-clapper testis: Some men have a testis that hangs loosely within the tunica vaginalis which is membrane around testis. Torsion can occur if the membrane is attached unusually high on the spermatic cord, often with a testicle which lies transversely rather than vertically. Testicle thus hangs like the clapper of a bell. Additionally, contraction of the cremaster muscle can then cause testicle to twist on it's cord.

Testicular torsion: Testicular torsion is a developmental abnormality. There is failure of the testicle to attach to the scrotal wall, also it can occur because there is a loose attachment of the epididymis to the testis, bell clapper deformity. Phantom pain can be relieved sometimes by injecting a long acting local anesthetic into the cord or using alcohol injection into cord.

Testicular torsion: Torsion of the testis refers to a twisting of the testis and spermatic cord resulting in a choking of the blood vessels to and from the testis. It is a true emergency as the testis can essentially die if not corrected within a few hours.  The condition requires a congenital abnormality of the mesentary suspending the testis in the scrotum.  As a result there is not much you can do to prevent.

It has to be differentiated from epididimoorchitis , an inflammation that also causes pain. An acutely tender painful testis is likely to be a torsion, particularly in boy before or at the time of puberty.

my testicles sometimes rotate 180degrees. they either go back themselves or i twist them back. no pain though. is this testicular torsion caused by bell clappers? or is this completely normal? i have read both and want a better answer. thanks?

: The normal testicle  can rotate 90-180 and  have some blood flow compromise. If this is happening frequently would follow up with urology as they may be able to tack the testicle down.

can an orchidopexy operation of testicular torsion by itself cause testicular atrophy? or is it caused by the no blood flow of the torsion?

: Usually the late intervention causes the testicular atrophy due to prolonged ischemia, not because of the orchiopexy procedure.
