Painful: Testicular torsion occurs when a testicle rotates, twisting the spermatic cord that brings blood to the scrotum. The reduced blood flow causes sudden and often severe pain and swelling. . It has to be differentiated from epididimoorchitis , an inflammation that also causes pain. An acutely tender painful testis is likely to be a torsion, particularly in boy before or at the time of puberty.

Always with pain...: Testicular torsion always comes with pain relatively proportional to the degree of twisting and subsequent sequential compromise to the lymphatic, venous, and arterial circulations and its resultant swelling and degree of tissue injury with ischemia to necrosis.
