Not always: Colitis can cause abdominal pain, diarrhea, bloody stools and sometimes rectal pain.  what other symptoms do you have?  Typically it persists for period of time i.e. not intermittent.

The primary cause of hemorrhoids is increased abdominal pressure, from chronic constipation, prolonged sitting, pregnancy and in some cases, from other diseases, such as liver disease. They are usually fleshy and soft but cause discomfort when engorged and bleed with trauma such as passing hard stool or hard wiping.

Dumb luck: Maybe a quarter of women of reproductive age have fibroids in the uterus that cause more or less trouble. There are a variety of ways to handle these nowadays, some letting you keep your uterus if that is your desire.

Lots of stuff: How long have you had the symptoms?  Brand new?  See your doc asap. Could be diverticulitis, appendicitis.
More chronic? Could be spasm (proctalgia fugax-more common in uptight men), more common stuff includes anal fissures (tears), hemorrhoids, inflammatory bowel, also tumors, isolated rectal ulcer; less likely diverticulitis, or appendicitis.  Could be gyn issues
