Kneecap tracking.: It may be related to quad muscle shutdown, following surgery, which can alter the tracking of your kneecap, and give you the "popping" that you're feeling. Check with your surgeon to see if that's the case. If so, it should get better with physical therapy.

Cartilage damage: You probably have degeneration of cartilaginous components in the joint. 
The most common culprit is the meniscus.
Diagnosis is done by mri, and arthroscopy provides the needed clean up.

recently had knee surgery to clean up soft tissue as well as remove plica.  surgery was 3 weeks ago and now knee catching again.  what should i do?

See your surgeon: If only plica excision and nothing else in the knee it can still pop and click if a little swollen. Stationary bike would be good for you. And check in with your surgeon

Hard to say: Sometimes, with time and/or therapy, surgery can be avoided. If you keep doing exercises and don't reinjuer it, often you can be symptom free for years. Sometimes, though, it can act up without warning.  If it keeps giving you trouble or acts up too often, then surgery may be a good choice. Discuss options with your orthopedist.

can putting ice on my knee reduce effusion? the swelling does not seem to down my patellar causes catching what does that mean? please help and explai

Mechanical knee: Mechanical knee symptoms with effusion is a meniscal tear until proven otherwise .See orthopedist for exam  whil ice will reduce unflammation it wont fix the non smooth gliding surface.
