It is a challange: It is a challange but can be done. It requires the aid of a good assistant and many times smaller sized instruments. Often you need to let the patients rest so they can help in keeping there mouth open as wide as possible. 
Often i will schedule more time for a person with a small mouth or someone with opening issues. 
Ask your dentist to let you take some breaks during the procedure.

Splinter: Depends on the situation.  The splinter definitely needs to come out.  If it is relatively small and is partially sticking out and the child is not hysterical or in distress, then you could restrain the child and try to pull the splinter out.  Then report to the doctor.  Otherwise it is best to get the child to the emergency room for a doctor to examine the baby and remove the splinter.

Apthous ulcers: Although the reason for developing these lesions remains a scientific mystery, they are small idiopathic lesions found in the softer folds of  mouth tissues. There can be some tingly feeling in the area prior the arrival, and about   3 days after they erupt for them to disappear.  Changes in weather, diet, and stress have been know to increase the chances of development.

I do not know: If your mouth is truly dry and you are having trouble producing saliva, then see your Dr to be examined and checked for underlying causes like Sjogren's. Sjogren syndrome is an autoimmune disorder in which the glands that produce tears and saliva are destroyed. This causes dry mouth and dry eyes.
