Fungus?: Although white spots could be an early manifestation of toenail fungus it could also just represent trauma to the nail. Give it some time. If it grows out you have nothing to fear. If it becomes thick and yellow see your doctor.

I would have the : Nail tested to confirm it is indeed fungus and if so and there is no contraindication recommend oral lamisil (terbinafine).

Pills or creams: Oral antifungals work best. Creams and lacquers do not work very well.

Depends: If you are treated for the fungus by an oral medication it can take up to 6 months for the nail to fully grow out. The nail may grow out flat like your normal nails or it may still have some thickness to it.  But taking the medication is no guarantee that you cannot get another fungal nail infection in the future.

No fast way: It takes 8 to 12 months for your nail to grow out. That said in my office we find laser to be the best.

It should: The nail looking normal is usually the sign that the fungus has resolved. However, there are other causes for thick nails, including circulation problems, psoriasis, and others. A person can have thick toenails without a fungus causing the problem.
