Time: Smoking does considerable damage and this increases with the time smoked and the daily number of smoked cigarettes.  When you stop,  your body has to adjust its metabolism, clean out toxins and regroup.  This takes time and it is years that statistics of the smoker match those of the never smoked group.  This is a noble goal however; the sooner you stop, the better.

Stroke: Smoking causes vascular contraction and can decrease cerebral blood flow. Stopping cigarette smoking is very important to preventing future strokes, progression of existing stroke worsening of underlying vascular disease.

Unlikely: Since smokable heroin use is so immediately reinforcing, and the dose of smoked heroin would be difficult to control. Methadone or Buprenorphine are often used for detox; Clonidine and tramadol are other options.
