Proctologist: I recommend consulting a proctologist for diagnosis, treatment and proper follow-up care.  You may be dealing with inflammation of the rectum or internal hemorrhoids. Anal fissures usually cause discomfort on defecating. Has a gastroenterologist evaluated your chronic diarrhea? A fodmap diet may be helpful in irritable bowel syndrome.

They usually don't. : The reason is that internal hemorrhoids don't typically cause pain on their own. If you have hemorrhoids and are experiencing pain then either what you think are internal hemorrhoids are really external or there's something lead going on at the same time like an anal fissure or ulceration. Best to have a through exam with anoscopy to be sure.

Menstrual pain: Severe menstrual pain with diarrhea may be due to endometriosis.  Oral birth control will control the effects of endometriosis (if no contraindications exist) and preserve fertility.  Oral bcp's will stop ovarian function and lessen the pain. See your gyn physician for an exam and treatment.

Hemorroids: Exam under anesthesia, the first part of rectal surgery when non-operative care isn't working. While you are comfortable under ga your surgeon examines the rectum. Prolapsing hemorrhoids are surgically treatable. Be well.
