One relevent question is if you are voiding a higher volume or urine overall; or just small amounts more frequently, implying inability to fully empty bladder. Your answer may be more complex depending on setting.

I : I am so sorry to hear about your wife's difficult situation. My advice is to seek the care of a pain management specialist. This is a doctor that is trained to treat pain using the least amount yet most effective pain medications and/or procedures. Often times doctors are in a rush and don't have adequate time to deal with the complexities of pain management therefore she will be much better under the care of a pain management specialist.
I hope your wife finds help soon.

Pain medication: Narcotic pain medication should be used for short periods after an injury or surgery, days to a week or two.  Try to use less pain meds every few days, follow up with appointments for your doctor or rehab if prescribed. Change to non-narcotic pain meds as soon as able. Take pain meds to make pain tolerable. If you take the medication to get rid of all pain, all the time, addiction is probable.

Safe pain meds: The safest is tylenol (acetaminophen). Narcotics used properly are safe.  Nsaids like Motrin allege and Aspirin are risky.

Different: I assume were talking about opioids. If that person is been accustomed to the high dose, you may see very little in the way they act. Especially if they are only taking opioids. If they were having side effects though, you would expect to see sedation, clumsiness, lack of interest, long periods of just staring. Very tiny pupils, dry mouth and sweating are also common.

Check testosterone: It's fairly common that chronic use of opiode derive pain killers like oxycodone and hydrocodone cause a low testosterone level. This can lead to loss of libido. If he can't get off the meds, check a testosterone level and if low his doctor may replace it.
Another thing to think about is chronic pain can cause depression and that can also cause libido loss. His doctor can help with that too.

Ibuprofen 800mg: Every 6 hours (unless you have a contraindication), sleep slightly elevated, switch over to moist heat application after 48 hours.  Sometimes steroids are given intra-operatively and even post-operatively if significant swelling is anticipated.  Normal body response to surgery.  Also just has to "run its course".

Possibly not: Drug testing can detect even minute amounts of painkillers and other drugs.  If your job requires drug testing, you need to find out which pain medications are ok:  tylenol (acetaminophen) or advil probably are, but vicodin or other narcotics even in small amounts probably are not.  Consult your pain management doctor for more advice, and help in cutting down on any medications that could cause you problems.
