i had a maxillary sinus surgery 4 months ago and about a got to have very dark circles under and inner sides of both my eyes, can they be related?

It may be...: It may be a result of congestion/swelling as a result of the surgery, however, 4 months is a very long healing period.  My suggestion is to have a follow up visit with the surgeon to ensure you are healing as expected.

Not : Not enough information without an exam to fully answer your question.  What do you mean that your sinuses are "free"?  Do you have nasal congestion?  Did you have an injury to your nose?  Are your eyes involved in any way?

There are multiple sinuses in your skull, above the nose and eyes, below the eyes, and between the eyes.  They all drain into the nose. Any one or more of these sinuses can be affected with either sinusitis or an infection leading to pain and tenderness to touch.

I recommend that you schedule an appointment with your physician for evaluation.

Sinus cyst: Cysts or inflammation of a maxillary sinus can cause cheek discomfort and headaches but double vision would not be usual. An otolaryngologist(ear,nose and throat specialist), opthalmologist or neurologist could advise you further.

B12: Unlikely but if the b12 is low it needs to be treated. Other causes include allergies and headaches such as migraines

Osteomeatal narrowin: CT scan finding only as you state. This is on the medial wall of the maxillary sinus and must remain patent for sinus ventilation.

Two different bones: The inner ear is located in the mastoid bone, which is behind your ear on each side.  The maxillary sinuses are in the maxilla, which is the cheekbone on each side.  While they are only six inches apart, they are not really close

Probably nothing: This is an incidental finding from your imaging study likely. It probably has nothing to do with your concussion. If, however, you are having sinus problems, i would follow up with your pcp.

Sinusitis with polyp: Your CT shows you have a sinus infection with polyps. Follow up with the doctor who ordered your test for clinical correlation.  Always the best person to interpret any testing is the person who ordered it.  They have all you history, exam, etc.

Possibly: Thickened lining of the maxillary sinus could mean inadequate drainage of your sinuses.  The increased pressure can cause headaches.  You need to be evaluated by an ENT doctor and possibly have a ct scan done for a complete diagnosis.

See an ENT: I agree you should see an ENT doctor, but 2mm is awfully small for a sinus cyst, and might not even show up on a ct. Are you sure it's 2mm and not 2 centimeters? That would be more common. And, this may or may not be the cause of your eye pain. You may also need to see an ophthalmologist for an eye exam. At any rate, surgery to remove a sinus cyst can usually be done endoscopically through the nos.
