Hand controls: The tens is adjusted by a hand held controller which usually contains the batteries.  You may need a little trial and error.  Our office uses the national company emsi which has sales reps that will personally instruct the patient.

Case by case: This would be decided on a case-by-case basis.  Your surgeon consultant would be able to recommend or not, after speaking with and examining you.  If hemorrhoids are thrombosed (clotted) and painful though, surgery might well be recommended.

Need echocardiogram!: A supracristal vsd is quite uncommon, making up only about 3% of all ventricular septal defects.  A small defect may not cause any signs of congestive heart failure.  The decision to surgically close rests on any interference of the function of the aortic valve by the shunting of blood through the hole.  This can cause damage to the valve leaflets, and leakage of the valve itself.

Unlikely: Unlikely due to avoiding viral or bacterial respiratory post op complications.
Also post op coughing with a cold can be painful and disruptive!, 
non emergent generally is delayed til recovery.

Thoracic spine: This region is difficult to access unless the chest cavity is entered. With thoracotomy, incision into the chest, most common complication is irritation of the nerve between the ribs (intercostal) and pain. This can be worse than the pain being treated in the first place. So, surgery is often withheld unless pain is severe or spinal cord function is compromised.

Muscles: Muscle tears do not lead to paralysis. Injuries to the spinal cord lead to paralysis.  Sounds like you'll be ok.

Pancreas: I don't know what you mean by "pancreas swelling". I might be able to advise if i had more information.
Regarding the kidney stone there are several ways to take care of it that might not require surgery.
You also need to know what is in your 24 hour urine collection to get an idea of why you formed a stone. That way you can usually prevent an occurence.

Probably.: Probably, if your orthopedist recommends it. Sometimes more damage can occur if it's left untreated.

Asymptomatic galston: Gallbladder stones or calculi are common 
Although larger one are usually asymotomatc 
Because they can’t pass thru to the biliary duct.
