Chest pain possible: Hiatal hernias can certainly cause chest discomfort but I would think palpitations would seem less likely to be directly related to the hernia. You are doing the right thing by having a stress test and would certainly do any other cardiac tests recommended.  If you believe you are having more severe symptoms or new ones I would seek emergency attention.

Possibly: Discomfort under the left rib cage may come from hiatal hernia but may also be due to other things such as gas entrapment in the upper left colon or even from pinched nerves in the rib cage or problem with the base of the left lung. It's best to have your physician examine and advise you,and if he/she feels it's advisable a referral to a gastroenterologist will be made.

Possibly: A traumatic event that causes a rib injury could cause a muscular rent, which is , by definition, a hernia. A bulge on the side of a rib injury could also be due to some muscle laxity, which could be contributed to some degree of denervation associated with trauma. This can also be caused by herpetic infection (shingles.) a good physical exam can define a hernia. Good luck.

Might feel like knot: Your hiatal hernia might feel like a knot under your ribs due to pressure. Keep a diary of your symptoms, knot, pressure, acid in your throat and mouth. Add to your diary when and what you ate, to see how eating affects your symptoms. Your diary will help your doc decide your best therapy. Most hiatal hernias are small and manageable with diet, medical therapy and scope/egd.

Incarcerated hernia: A right sided paraumbilical hernia can occassionally incarcerate a portion of a loop of small bowel.  Initial inflammatory changes could cause refetted pain to the rib areas that you describe.

No.: Usually, an umbilical hernia does not hurt. The worst-case scenario would be if a loop of bowel gets caught in the hernia -- that hurts! This left-sided abdominal pain may be from abdominal muscles being strained or some colon problem..
