Ear fluid: Allergy may cause this since sinus congestion may lead to swelling of the tube leading to the inner ear. Also,  possible is an upper respiratory virus.  Usually, this type of fluid can be improved with decongestants.  Sometimes,  prescription medication,  if it persists longer than a few days.

i have ringing in the ear after using the neti pot. doctors see no fluid in ear.  ct and mri have both come back normal. what else can i do for this?

See specialist: Perhaps you have a viral process affecting the inner ear such as a labyrinthitis. I don't know what kind of doctors you are referring to who have seen your ct ; mris studies but if the condition continues perhaps you should ask to be referred to an audiologist to test your hearing to make sure that it is intact if nothing else seems to be amiss.

Not usually: Fluid in the ear does not generally cause vertigo.  I've seen patients complain of imbalance with fluid in the ear.  The most common causes of vertigo are meniere's disease, migraine, benign paroxysmal positional vertigo (bppv), viral labyrinthitis, vestibular neuronitis.  Fluid is not seen in the middle ear with any of these, generally.  Fluid changes in the inner ear cannot be seen on exam.

: Allergies cause a localized histamine response which may lead to localized fluid accumulation. The lymph nodes are useful in draining excess fluids, foreign material as well as coordinating localized immune response. Slight swelling may be due to fluid clearing or immune response to any exposure (viruses, bacteria, trauma) in the head region. (not just the ear)

: There is no lymph node in the middle ear where the fluid is. The increased pressure in the middle ear causes your pain. When the inflammation comes down with medication, fluid is reabsorbed and drained out, pain is relieved.  You can take Tylenol for pain as needed besides pill and inhaler for the allergy.

Serous otitis media: Sounds like serous otitis media. A non infected collection of fluid behind the eardrum. This is why various antibiotics haven't worked.  May want to have an ENT doc look at it to confirm the diagnosis and consider treatment vs continued watching.

No: Lower molars may refer pain or other symptoms to the ear so see your dentist for evaluation.
