i had open heart surgery to replace my auortic valve. now they are saying i have cystic medial necrosis. does this mean i have marfans?

Possibly: One of the features of Marfan's (a disorder of abnormal collagen tissue formation) is cystic medial necrosis, leading to abnormal tissue within the layers of the aorta which can lead to anneurysm formation of the aorta (a risk for regurgitation, rupture, and dissection).  When such an aortic root measures more than 50 mm in diameter, these risks increase and require aortic root repair +/- avr if ai.

i have a visible carotid pulse in the right side of my neck, dr says sounds like a bruit sound. bp is good, and heart sounds good. is there reason for concern?

Cysts in bone: Cystic changes or erosions in bone post trauma can indicate that the bone matrix was disrupted from the injury and cystic changes have resulted.  It does not mean that you have avascular necrosis, but it can mean that there are changes in the trabecular patterns of the bone due to the trauma. Be happy!: At least a serious problem such as avn or significant osteoarthritis exists.  Your pain may be coming from trochanteric bursitis or your tensa fascia latus. See someone in orthopedics, physical medicine or rheumatology!

Accepted Answer

Yes: Some murmurs can sound like bruits and the noise radiates up to the neck, especially those involving the aortic valve.  Any new murmur or bruit should be evaluated further by an ultrasound of the heart and also sometimes the carotids can be checked by Ultrasound too.  How serious the murmur is depends on the symptoms you are having, of course.  There are many murmurs that are loud but no problem.

Vibration sounds: Bruits: vibration sounds made when arterial flow is restricted by stenosis. The obstruction causes an increase in velocity of the flow similar to the way that water flows faster from the end of a hose when you restrict the flow with your thumb. Or, water flow over rocks makes a lot of noise. Water flow deep and without obstruction is quiet.  Physicians listen for bruits over the carotid arteries.

Yes: Bruits can change their sound depending on the degree of blockage. In the carotid arteries, for example, as a blockage is almost complete ( > 90%), the bruit actually goes away! this is why a bruit is not the best way to evaluate someone for carotid disease. An ultrasound is much more accurate!

See your doctor: This may be early sign of carotid disease.  As the carotid narrow blood squeeze through a tight area faster resulting in a bruit that we hear.
