No, but unusual: A carotid bruit is a sound heard when a stethoscope is placed over the artery in the neck. It is caused primarily by blood flow turbulence - like water in a brook running over rocks. Turbulent flow can be caused by atherosclerosis, aneurym, injury, and congenital abnormalities. It can also be heard by abnormalites in the proximal arteries and heart valves. It should always be evaluated.

Yes: A carotid bruit is due to turbulent flow from a blockage in the carotid artery. The natural course of carotid disease / atherosclerosis is progression over time. This is especially true if people still smoke. When the blockage in a carotid artery becomes near occlusive, the flow rate drops and the bruit typically goes away.

Yes: Trick question, but here are my thoughts.  A carotid bruit is due to a partial obstruction in the carotid artery.  A "sub-clinical" lesion may only give rise to a bruit during periods of intense exercise, when cardiac output is high.  In the resting state, a bruit may not exist.  At any rate, a carotid bruit in any circumstance should be evaluated.

See your doctor: This may be early sign of carotid disease.  As the carotid narrow blood squeeze through a tight area faster resulting in a bruit that we hear.

Carotid bruit: the tests you had would document if there was something abnormal, as they didn't  what you have is not serious. Often as people age there can be tortousity of blood vessels that can emanate a bruit.
If concerned repeat scan of carotid in about six months.

This is an emergency: You need to get to an EMergncy room right away. please call 911.  This could be a carotid dissection, or aneurysm. Either way you do not want to jeapordize blood flow to your brain in any way.  Call 911 immediately, expecially with headaches and such high BP.

Headache: Severe refractory headaches referred as worst headache of my life, specially if abrupt onset, may suggest a very serious condition (brain hemorrhage). Migraine, the most common form of  primary  headache disorders can be severe.  Other conditions can cause headaches. Sinusitis, and trauma for instance may also cause severe haedaches. For proper diagnosis and treatment i suggest medical evaluation.
