Alot going on: The palpitations are likely to be related to your heart. The back pain is atypical for heart disease. Leg pain is never related to heart disease. Weakness is a non-specific symptom caused my myriad issues. Shortness of breath should be further evaluated - it requires a detailed look as there are many causes.

Limited to bowel: Technically speaking, IBS-C (constipation predominant) shd only affect the bowels. IBS isn't a systemic disease like IBD (inflammatory bowel disease - Crohn's or Ulcerative Colitis). That being said, IBS can certainly cause anxiety / panic disorder (it has a way of affecting the mind, sleep, &amp; there4 personality), which can then explain the symptoms u describe. Micro-hematuria: Microscopic hematuria is fairly common and often from a benign familial tendency.  Unfortunately, blood in the urine is also often the only sign one gets from some conditions that need to be discovered, like bladder or kidney cancer or kidney stones.  See a urologist for workup. It likely will involve an office cystoscopic exam and upper urinary tract imaging.

: A Cardiology referral is often helpful. This may be ordered by your physician. Please see the Emergency room for evaluation if unwell. Evaluation for Flu and COVID-19 recommended. Watch for signs of shortness of breath and fever.

: It would be helpful to know your hemoglobin, hematocrit, iron, ferritin levels. But, it sounds like you have anemia and you are having symptoms from that. Are you bleeding from some place? Do you need supplemental iron? You need to be evaluated ASAP. If bleeding, it needs to be stopped. You may need a blood transfusion depending on your red blood cell count. You should go to the ER for treatment.
