: Long COVID-19 may cause a variety of chronic and persistent problems.  A true wear and tear arthritis would be unusual for your age, and coincidental lupus has no relation to the virus.  Potential of fibromyalgia and even myositis might be considered.  A small fiber neuropathy may act this way.  Start with your primary doctor, and perhaps see a physiatrist thereafter.

Yes: Some of the drugs used to treat rheumatoid arthritis can cause damage to lungs.  The most common would be the drug methotrexate.  This drug requires lung function monitoring with breathing tests/xrays because it can inflame the lungs (pneumonitis) or cause scarring of the lungs (fibrosis).  Other drugs for rheumatoid cause problems less significant.

Depends on lungs: It sounds like you have an overlap syndrome with an inflammatory arthritis as well as inflammation of your muscles and lungs. In this situation my experience has been that the biggest problem is often related to the severity of the interstitial pneumonitis. Relatively speaking, there effective therapies for both Rheumatoid Arthritis as the myositis so I think those will be well-controlled.

Breastfeeding in RA: Acetaminophen is regarded the safest medication.  Ibuprofen may be used but should be taken just after breastfeeding so that the lowest amount of the medication is excreted into breast milk.
